Ugliest of the usability problems

What are the ugliest of the usability problems (top two or three)?
One really stands out, I think:

  1. User registration in the pollwiki, where the username is the
     mangled email address (e.g. Mike-ZeleaCom).  I don't know if
     anybody's ever got that right, on the first go.

Is anything uglier than that?  Or what's #2?
